MEDICARE ANNUAL WELLNESS The goal of OCM is to keep your seniors aging in place. We specialize in the Annual Wellness Visit. We provide comprehensive and efficient AWVs. There is no concern over transfer of care or having to change providers. We do not diagnose nor treat ailments. Any foresight concerning extreme situations, we will contact the immediate administrator. Patient care is OCM’s top priority but we also care about you, as caregivers. From our Owners to our Direct Supervising Physician and staff, we are an Oklahoma grassroots company. We are the only local company currently specializing in and servicing state-wide Medicare prevention care. Initial and Subsequent Assessment: This assessment is covered 100% under the insured’s Part B plan. The assessment is conducted in a 12-month interval. The initial assessment should be completed 12 months after the insured’s “Welcome to Medicare” physical visit, provided by their physician. The subsequent assessment is completed annually thereafter. Either assessment can trigger secondary questionnaires, as follows: Depression, Smoking Cessation, Alcohol Substance Abuse, Advance Care Planning and Weight Loss Management. Through a simple one-on-one conversation, we can combat “white coat syndrome” and possibly discover new information about your insured

This secondary questionnaire examines the shifting level of depression or will

discover unknown depression symptoms. It is our goal to report new information to

the immediate care giver so that an updated or new plan of care can be established.

This secondary questionnaire examines the insured’s knowledge of their future health care. The importance of planning is expressed and documents are given, if requested. Many insured individuals either do not have a plan in place or have an outdated directive.

This secondary questionnaire examines the need or desire to cease smoking or using tobacco, of any sort. Although most senior locations are “smokefree”, this assessment can help identify the dependence and smoking triggers. It is our goal to report new information to the immediate care giver so that an updated or

new plan of care can be established.

This secondary questionnaire examines the need or desire to cease alcohol and/or drug addiction, of any sort. Although most senior locations are “alcohol/drug free”, this assessment can help identity the dependence and substance triggers. It is our goal to report new information to the immediate care giver so that an updated or new plan of care can be established.

HOW IT WORKS • OCM staff will gather pertinent information from your location, with minimal interruption to your day-to-day operations • OCM staff will verify insured’s eligibility against specific Medicare codes for the AWV and additional free screenings • OCM staff will perform the AWV for each insured via a comprehensive and proprietary questionnaire, which is conducted in under 15 minutes • OCM staff will provide any necessary documents for educational purposes 
• OCM staff will offer an optional printout for the insured or insured’s medical chart, including a free screening recommendation list.
WHO WE SERVICE AND HOW OCM is contracted with Medicare specifically to work in conjunction with your current provider. Since most insurance companies have their own Wellness Programs, OCM focuses on original Medicare insured individuals. OCM services all senior locations such as: assisted living facilities, independent living facilities, community centers, aging services centers as well as independent provider offices. Our services come with a Certificate of Participation once services are completed. This will show your state and federal agencies, your initiative in going above and beyond for your insured’s care. OCM uses encrypted software, encrypted email and a proprietary, comprehensive questionnaire. We are HIPAA compliant and fully contracted with Medicare. Use our passion for your patients. Jeanette “Jeanie” McCollom - Founder Owner Jeanie was raised in historic Guthrie, Oklahoma and graduated from Guthrie High School in 1992. She has continued her progress towards a Bachelors in Business Management at the University of Central Oklahoma. Jeanie has an extensive background in accounting, medical and pharmaceutical auditing and credentialing and as a AWV Specialist, just to name a few. Jeanie is no stranger to high level, demanding management positions such as Sole Accountant at a top financial firm in OKC, Director of Operations at a Fortune 500 infusion pharmacy, AWV Director for a top Edmond physician, Director of Business Development for a home based primary care physician office, all of which benefited from her outstanding work ethic. Some of the projects she has implemented are: numerous software conversions, training staff, streamlining policies and procedures, developing workflow manuals, marketing and developing business models, originating and executing entire AWV programs for physician practices, in the metro area. Jeanie’s goal for Oklahoma Clinical Management is to help build a better relationship between patients and physicians. Putting all of her knowledge and expertise to work for you, she can help extend your Quality of Life and Quality of Care practices, to all Medicare patients. She wants to harmonize preventative care with efficient processes so that it is a winning situation for all involved. Her compassion and knowledge are unsurpassed.
William “Cody” Turpin - Founder/Owner Cody is a lifelong Chickasha resident. Cody attended school in Chickasha and graduated from Chickasha High School in 2002. After high school, Cody attended Southwestern Oklahoma State University. After college the corporate world came knocking, and Cody took the job of Corporate Pharmacy Trainer at CVS/Pharmacy. Though he loved the travel, Cody was anxious to get back to Oklahoma with his skill-set. In 2009, he took a job with a local infusion pharmacy in Oklahoma City. There, Cody quickly rose through the ranks, and soon took over the operations for the Oklahoma City location. A few years later, he was afforded the opportunity to take over as the Director for another infusion pharmacy in the Oklahoma City market. In this capacity, he was able to display his unique talents of assembling a stellar team and leading the group through unprecedented growth for the company. While Cody enjoyed this leadership opportunity, what he knew was that he could help more people if he started consulting to reach more people in the healthcare market. That leads us to today. Through consulting with different companies and staying in close contact and working with a previous co-worker, his now partner Jeanie McCollom, they together started Oklahoma Clinical Management. Specializing in Medicare Annual Wellness. Cody’s genuine passion is patient focused care. Through his own experiences with family and friends, Cody understands the importance of taking care of the patient first and staying patient focused throughout the patient’s healthcare journey. Cody’s number one goal for Oklahoma Clinical Management is to provide the best patient focused care possible, and hopes to work with as many families in Oklahoma and beyond in the years to come to provide this kind of care.
Brian McCollom - General Manager Brian is a lifelong resident of Oklahoma City. After graduating high school in 1994, Brian studied computer science at Oklahoma City Community College. After college, he started working in the printing industry as well as marketing coordinator for a large Oklahoma City based heat & air company. In 2001, Brian became a store Manager for Quik Print of Oklahoma City. He spent 18 years with the company and was very successful in turning his branch in to one of the highest producing locations in the city. He developed outstanding leadership skills as well as delivering the highest level of customer service to clients. Although Brian was happy with his success, he welcomed the opportunity to join the team at Oklahoma Clinical Management. He could see the success that OCM was having and the great service that they provide to Medicare patients and doctors. Brian joined the OCM staff in June of 2019 as the General Manager. He brings with him skills in marketing, leadership, management, and IT. His goal is to help OCM continue to grow and help as many people as possible.
Javi Turpin - Operations Coordinator Javi Turpin comes to Oklahoma Clinical Management as the Operations Coordinators. In his free time, javi enjoys spending time with family and traveling to new places. Since graduating from Putnam City West in 2013 javi studied Nursing at OSU-OKC. After college javi has been employed in the operations of senior living facilities in the Oklahoma City area, Javi enjoys working and engaging with the aging population that OCM services. We look forward to Javi’s continued growth with OCM and can’t wait to see what the future holds.
